What actually failed

If the mismatch is isolated to one sensor and a second independent method agrees with the process, you still hold until the first sensor is either replaced or shown to be within its calibration interval with a documented as-found. If two methods disagree, hold the material and the methods. If the record is missing, hold as if the check failed.

Interview notes

Ask the operator what they did with their hands in the five minutes before the alarm, not how they feel about the equipment. Ask QA which report they already promised to a customer. Those two answers tell you whether the hold will leak.

Sampling

Sample from the edges of the hold, not only the middle. Edge units see the transition. If edge units fail, expand. If they pass, do not shrink the hold until you can explain the original signal.

Documentation language

Use complete sentences. Name files. Put times in 24-hour local with timezone. If you write approximately, add the source of the estimate. Reviewers treat vague time as missing time.

Release criteria
