Jacket Temperature Holds is a hold event, not a hallway conversation. The clock starts when the first person who can stop the lot sees the mismatch, not when the form is typed.
What actually failed
On a chemical line, the failure is almost never the instrument alone. It is the gap between a passing check and a later signal that the check no longer describes the material still in process.
Write the first sentence of the record as an observation: what was seen, on which asset, at which time, against which limit. Do not open with a cause. Cause comes after you know which units, lots, and data files are in scope.
Hold boundary
Hold means physical segregation plus electronic status. A verbal pause that leaves units on the same rack as released product is not a hold. Tag the rack, lock the status, and name the person who can lift it.
Define the unit of hold before anyone asks how much product is involved. Chamber, sequence, bag, mixer, pallet, panel, bomb, stencil, tray: pick one and stick to it until sampling proves a tighter cut.
Evidence pack
- Raw instrument file or chart, not a screenshot of a summary screen.
- Last passing check with time, operator, and standard used.
- Identity of material still in the affected equipment.
- Cleaning or changeover status of the next lot queued.
- Names of people who already saw the data unofficially.
What not to do in the first hour
Do not re-run the check to make the chart look better unless the procedure already allows a documented repeat with the first result kept. Do not blend passing and failing files in one PDF. Do not let production 'keep going on the next SKU' on the same equipment until cleaning status is proven.
Decision table
If the mismatch is isolated to one sensor and a second independent method agrees with the process, you still hold until the first sensor is either replaced or shown to be within its calibration interval with a documented as-found. If two methods disagree, hold the material and the methods. If the record is missing, hold as if the check failed.
Interview notes
Ask the operator what they did with their hands in the five minutes before the alarm, not how they feel about the equipment. Ask QA which report they already promised to a customer. Those two answers tell you whether the hold will leak.
Sampling
Sample from the edges of the hold, not only the middle. Edge units see the transition: vacuum break, temperature overshoot, first vials after a failed suitability, first boards after a stencil wipe. If edge units fail, expand. If they pass, do not shrink the hold until you can explain the original signal.
Documentation language
Use complete sentences. Name files. Put times in 24-hour local with timezone. If you write 'approximately', add the source of the estimate. Reviewers treat vague time as missing time.
Release criteria
Release needs a written why-this-lot-is-still-the-lot-you-intended. Matching a spec after extra testing is not enough if the extra testing was invented after the failure. Either the protocol already allowed the test, or you are in deviation territory with QA ownership.
Training residue
After the event, the useful training is a five-minute walkthrough of the actual tags and screens, not a slide deck. Show where the hold status lives and who can change it. People copy what they last saw work.
